IF YOU LIVE MORE THAN AN HOUR FROM THE CENTER, call us at 214.824.6181 and ask us to email a five page document which gives some major information you need to consider. Also, you are encouraged to read The Cross and The Switchblade by David Wilkerson (Founder of Teen Challenge).

Step 2: The Application
Complete the application. Follow through on all Entrance Requirements listed in packet. If you need clarification on any part of the application, call us at 214.824.6181.

PLEASE NOTE: STEPS 3 AND 4 SHOULD BE DONE ONLY AFTER THE PERSPECTIVE STUDENT MAKES THE DECISION THEY ARE DEFINITELY INTERESTED IN COMING TO TEENLIFE CHALLENGE.

Step 3: The Physical Examination
We require a complete physical examination by a doctor at your expense. The minimum level is a “sports physical” that is commonly used for local athletic programs. They are inexpensive and available through many physicians.

Step 4: The Blood Test
Blood tests for Basic STD Panel, HIV, Hepatitis Panel (A, B & C) and TB are required and may be obtained from the clinic of your choice. Sign Release of Medical Forms at Doctor’s Office so they can FAX results to 972.278.8003. We need BEFORE you come into the program.

Step 5: Entrance
After all of the above STEPS are completed, within 24 hours of your readiness to actually come into the program, call 214.824.6181 to schedule your definite date and time of intake into the program.
